2011 Open Regstration and Uniform Fitting

During Open Registration and Uniform Fittings, parents will fill out all necessary paperwork for participation. Registration payments will be made at this time. Families that have multiple children that are participating will have discounts available to them. Payment plans are available. Registered children will need to be present in order to be fitted for their uniform. Parents will need to provide a copy of the child's birth certificate and a current sports physical for our records. Parents/children that cannot attend one of our scheduled dates should contact the football or cheerleading director to schedule a seperate registration and fitting. Any late registrations may be subject to a $20.00 late fee.

Please download, print, and fill out the registration form. Drop it in the mail along with your payment (checks only please), a copy of the child's birth certificate, and a copy of the child's current sports physical to the following address or bring it up to the practice fields during practice:

The cost to play is $99.50. Please make the check out to LVYF&C. If you need assistance in paying, we do have scholarships available.
